Deductibles and coinsurance count toward the out-of-pocket maximum. The in-network out-of-pocket maximum amounts for the EPO Network Plan and EPO HP-Network Plan are: • Associate: $5,000 • Family: $15,000 After you meet the out-of-pocket maximum, the EPO Network Plan or EPO HP-Network Plan will pay for 100% of all covered in-network expenses.
