Deductibles and coinsurance count toward the in-network out-of-pocket maximum. The out-of-pocket maximum amounts for the PPO Plan are: • Associate: $4,000 • Family: $8,000 After you meet the out-of-pocket maximum, the PPO Plan will pay for 100% of all in-network covered expenses. Note: The annual out-of-pocket maximum for out-ofnetwork benefits under the PPO Plan is higher than the in-network out-of-pocket maximum.
